Federal authorities are investigating the death of a Stoughton man who fell off a forklift and struck his head on the concrete floor in the Taunton warehouse where he worked.
Authorities say 51-year-old Alan Forsyth was taken to Morton Hospital and Medical Center at about 11:30 a.m. Monday where he was pronounced dead.
Police tell the Taunton Daily Gazette that Forsyth was operating a stand-up forklift inside the Super-Dog Pet Food Co. warehouse when it struck one of the building’s steel support beams and he was thrown to the concrete floor. Police ruled his death an accident.
The federal Occupational Safety & Health Administration is investigating.
