An Angie, La., insurance agent was issued a cease and desist order, summary suspension and served with a $14,000 civil fine notice recently for alleged misappropriation, document forgery and filing false public documents, the Louisiana Department of Insurance reported.
Kimberly McKenzie is alleged to have submitted two fraudulent license renewal applications to the department by not disclosing her prior criminal record. LDI also believes she misappropriated insurance premiums and altered or forged documents while conducting the business of insurance.
McKenzie is currently on three year probation for convictions including forgery, unauthorized use of an access card, issuing worthless checks and identity theft.
McKenzie was issued a life/health license in May 2001 and a property/casualty license in June 2001. Both licenses were suspended in the current action.
