Pennsylvania Insurance Commissioner Diane Koken has released enforcement actions and market conduct examinations taken in May against insurance producers and companies licensed to conduct business in the Commonwealth.

Michael P. McIntyre, Dallas, Texas – $1,000 fine and three-year license supervision for receiving commission with being appointed by the company;
Barry K. Taylor, Downingtown, Pa. – five-year license supervision for authoring non-genuine signatures of two policyholders on a life insurance application;
Jed A. Karpinski, Lancaster, Pa. – license revocation and $6,000 fine to one felony count of Conspiracy to Commit Mail Fraud;
Ira Henderson, Jr., Avalon, Pa. – Although revoked under a previous Consent Order, the Department has agreed to license Henderson with the stipulation that he will not engage in surplus lines insurance for a period of three years and is under a five-year period of license supervision
