A private plane was aloft for about a minute and flew three miles before crashing into a rented vacation home in Gearhart, Ore. last week, a preliminary report from the National Transportation Safety Board said.
The crash killed Julia Reimann, 10, of Portland, and her cousins, Hesam Farrar Masoudi, 12, and Grace Masoudi, 8, of Denver, pilot Jason Ketcheson of Cannon Beach and passenger Frank Toohey, 58 of Warrenton. There were three survivors.
The report said visibility at nearby Astoria was 2.5 miles with mist and clouds at 300 feet, requiring anyone flying in it to be instrument-rated, which Ketcheson, was.
The NTSB says it will take about a year for a cause to be determined and released.
