La. Agent’s Operations Shut Down

October 12, 2006

Louisiana Department of Insurance Fraud Investigators shut down all insurance licensing privileges of a Delhi, La., producer (agent) on multiple charges, Insurance Commissioner Donelon announced.

Janis Dalene Simms Blaylock, 29, was served by Louisiana Department of Insurance investigators with a cease and desist order and a summary suspension of her insurance license at 2385 Duckworth Road in Pioneer, La. Blaylock allegedly misappropriated almost $17,000 in insurance funds from customer premium payments for her own personal use and filed false claims on policies of unsuspecting policyholders to recoup the losses. Investigators allege that Blaylock submitted at least 14 false insurance claims between August 2005 and January 2006.

Department records show that Blaylock worked as an insurance agent in Delhi and vicinity. She held a Life, Health and Accident and a Property/Casualty license from 1999 until the current action.

Commissioner Donelon urges consumers who have any questions regarding policies Blaylock sold or serviced to call the Department of Insurance Fraud Unit at 225-342-4956 or 1-800-259-5300. If callers don’t want their names used, they can request that their involvement be kept confidential.
