Former La. Insurance Agent Arrested on Fraud Charges

The Louisiana State Police Insurance Fraud and Auto Theft Unit announced the arrest of a former Iberia Parish insurance agent on charges of Insurance Fraud and Forgery.

Troopers believe David Sampy, 43, of Sugarhill Drive in New Iberia, altered the insurance applications of four Terrebonne Parish School Board employees during the 2004-2005 school year. Each person’s application allegedly had been altered by Sampy to increase the previously agreed upon premium amount, thereby increasing his commission. In one instance, Sampy forged the client’s initials to changes without the client’s knowledge or consent, the LSP said.

Sampy had been a licensed Louisiana producer until October 2006 when the Louisiana Department of Insurance issued a Cease and Desist Order ordering him to stop conducting business.

Sampy was arrested and transported to the Terrebonne Parish Jail where he was booked on four counts of Insurance Fraud and one count of Forgery. His bond was set at $100,000.00.
