The Occupational Safety and Health Administration has cited two companies for a fatal accident at a plant construction site in Chippewa Falls.
OSHA issued what it calls a “serious” citation, in which it’s likely the companies knew or should have known about the potential hazard.
Thirty-six-year-old Kevin Kadrilick was killed and three other workers were injured when a wall collapsed at the EOG sand plant.
The concrete contractor, Lewis Construction of Schofield, and the general contractor, Cedar Falls Building Systems of Menomonie, were cited.
The Chippewa Herald says the companies have 15 days to contest the citations and could face tens of thousands of dollars in fines.
OSHA says the companies failed to provide adequate bracing for the concrete wall that collapsed. The workers were on elevated walkways attached to the framework.
