The Nebraska Department of Insurance issued a Cease and Desist order against Richard A. Brooks and Brooks & Associates Ltd., 1338 Park Avenue, River Forest, Ill., instructing Brooks to immediately cease transacting the business of insurance in Nebraska.
The department said it has reason to suspect that Brooks solicited the sale of commercial general liability insurance to Nebraska businesses or businesses traveling into Nebraska to conduct business by providing Certificates of Insurance for payment of a premium.
During further investigation, the department determined that insurers cited on the certificate were not insurance companies and/or the policy numbers provided were not legitimate policy numbers. Further, the department’s investigation revealed that Brooks is not a licensed insurance agent, agency or insurer.

The department recommends that businesses refuse Certificates of Insurance issued by Richard A. Brooks and Brooks & Associates Ltd., and require insurance coverage that can be verified.
