Kentucky Arena Floor Collapse Blamed on Human Error

The new Louisville arena’s construction manager says a concrete floor collapse at the construction site last month was caused by “human error.”

The Courier-Journal of Louisville reports that M.A. Mortenson executives said the collapse occurred after their concrete subcontractor failed to install 20 posts to support the concrete until it cured.

The Mortenson executives say at least one post was installed incorrectly and the subcontractor’s employees didn’t notice the error.

The comments were made Monday at an arena authority meeting.

Two of the three workers injured April 27 have returned to work. Mortenson officials say the third worker — who suffered a puncture wound to his arm — hasn’t returned yet.

A report from Kentucky’s OSHA office is expected by the month’s end.
