I think they got off cheap on these fines. I had a customer in CA fined nearly twice this for not reporting a fall from a ladder to OSHA. It didn’t result in anything more than a broken bone.

Yea, I would think that any OSHA violation that results in death should be fined a bit more heavely than that.
