Texas Comp Division Takes Steps to Secure Confidential Information

The Texas Department of Insurance Division of Workers’ Compensation announced that in the effort to increase the security of confidential information in the workers’ compensation system it revised the majority of its forms to require only the last four digits of the Social Security number.

The updated forms can be downloaded from the TDI Web site at http://www.tdi.state.tx.us/forms/form20all.html.

The nine-digit Social Security number will continue to be required on a few DWC forms:

–DWC Form-001, Employer’s First Report of Injury and Illness;
–DWC Form-021, Payment of Compensation Notice of Refused or Dispute Claim Interim;
–DWC Form-032, Request for Designated Doctor;
–DWC Form-041, Employee’s Claim for Compensation for a Work-Related Injury or Occupational Disease; and
–DWC Form-042, Notice of Fatal Injury or Occupational Disease and Claim for Compensation for Death Benefits.

These forms, sent to the agency by system participants, are often used by TDI to create a record of an injury or illness that may not have already been reported.
