Texas DWC Posts Updated Forms and Notices Online

Forms and notices used by Texas workers’ compensation system participants recently were updated to reflect the new administration of the system by the Texas Department of Insurance (TDI), Division of Workers’ Compensation. The revised forms are available on the agency Web site at: www.tdi.state.tx.us/wc/forms/index.html.

Use of the revised forms is mandatory on or after May 1, 2006, but system participants can begin using the forms immediately if they wish.

Employers in Texas are required to post notices to their employees as to whether or not they carry workers’ compensation insurance. The notices, in both English and Spanish, are provided on the website for convenient access for employers to download and post. Other forms and notices for employers, injured employees, insurance carriers and health care providers are available for downloading from the agency website, and paper copies are available from Division field offices and from Division Customer Services.

All forms and notices are available in Adobe Acrobat PDF format. Many of the PDF-formatted forms are interactive, allowing injured employees and other stakeholders to complete and print them from a computer screen. The DWC FORM-103, DWC FORM-105 and DWC FORM-109, and the Plain Language Notices are available as Word documents and in PDF format.

The DWC FORM-62, “Explanation of Benefits,” available on the website is bar-coded. However, the Division is exempting the form from bar-code requirements if it is developed by insurance carriers or other workers’ compensation system participants. The revision date also is not required on the DWC FORM-62.

As of Sept. 1, 2005, when the Texas Workers’ Compensation Commission (TWCC) was abolished, workers’ compensation system forms have been renamed. Forms previously titled in the format “TWCC-(Form #)” are now titled “DWC-FORM (Form #)”. The form numbers have not changed.
