Are the right kind of training being provided to insurance cos., employers, and health care providers to better educate them on disability management, workers comp, and other health care issues?

Margaret, in reality, your question is the root and source of the real problem. Insurance carriers and claims adjusters have taken control of TWCC telling doctors their medical decision making is wrong. Hence, deny claims, allow and pay insurance company attorneys begin the game of litigation. That is why doctors proclaim TWCC and insurance companies are “gaming” the system.

Along the same thought, employees & employers should be concerned by the recommendation of using “…qualified, credentialed medical doctors…” without defining what ‘qualified’ is. Without specific training and a certified continuing education process in place, Tx work comp opens the door for a ‘mill’ mentality. You will have created a GateKeeper MD, who is not trained in the Functional Assessment/ RTW model and is not directly involved in the Physical Assessment & Return to Work care of the employee, doing simple orthopedic/ provocative tests and then shipping off care to his ‘preferred providers’.

What TWCC should do is state that only physicians that have taken accredited courses on how to assess functional testing and implement effective RTW programs based upon evidenced based, objective testing will be allowed to see WC patients. It would not matter what type of physician the primary care physician is (MD, DC, DO- although I agree Dentist is over the top), the clinician would have to take a vested interest (ie. be certified and have an annual recert.) if he wanted to work in the WC arena. Both employer & employee would know this.

In addition, by following the MediCare model of showing monthly objective improvement for continued care, the review of the Drs. and clinics effectiveness could easily be audited. No more of these runaway cases that have ruined the WC environment.

The bottom line is open accountability- for the employer, employee, doctor, insurance company and most importantly, TWCC. If everyone knows the rules and the proper steps involved, you would think everyones jobs would be easier!

Doctors of Chiropractic are portal of entry healthcare providers and more than qualified to serve as the treating doctor for certain workers compensation cases.
Doctors of Chiropractic are licensed in all 50 states and recognized by the World Health Organization.
Doctors of Chiropractic receive their degrees from fully accredited colleges and universities; accreditation by the same regional bodies as for state universities.
The key, as for all specialties, is to manage only those cases which primarily fall within that specialty’s scope of practice.
It is equally ridiculous for a cardia surgeon to become the treating doctor for a sprain/strain musculoskeletal injury as it is for a chiropractor to become the treating doctor for a hand laceration.
Professional bias has no place in this argument/debate.
I dissected the same human cadaver, studied from the same core anatomy, diagnosis, physiology, pathology, and microbiology books, and had the same caliber of instuction from M.D.’s and PhD.’s.
There is substantial research that verifies that Doctors of Chiropractic treat musculoskeletal injuries more cost effective and with shorter off work times than allopathic treatment.
Doctors of Chiropractic offer not only alternative care, but quite often the most proper care and care not offerred by allopaths.
It is highly suspect to believe that the openly biased allopathic community will refer to Doctors of Chiropractic if given a choice.
This will be the ultimate detriment to the patient and reveals that it really is not their choice of treating doctor.
