Recently an PL ins co was instructed to add and delete a vehicle, common exercise. Took 9 endorsements to get it right. Apathy? Incompentence? Stupidity? Computer error? Half a$$ effort? Just don’t GAS?

Like most fatal tragic incidents, there were many contributing factors but attending personnel never should have let it blow out. They neglected all signs of events that happen all the time while drilling and did nothing to prevent the blow out and seemingly weren’t aware that it was blowing out until it happened. Asleep at the wheel. 11 men paid with their lives.
Sounds like a cascade of events with one common thread that allowed the first domino to fall. Human error in judgement, from the stock-holders of all the companies, from the workers who weren’t even aware of the things they didn’t know or understand about their own responsibilities, from management at all levels who probably contributed the match for Mrs Oleary’s lattern known commonly as human greed.
This amounts to a global indictment of human being to do a job well at all levels. People used to laugh at the TQM (Total Quality Management) concept that focused on processes and metrics to prevent failures, but now who’s laughing?
Those managers and executives involved should be summarily fired without severance for gross neglect. They’re the ones paid to make sure this didn’t happen. Now the PONC (price of non-conformance) is an astronomical amount that didn’t have to be incurred. This kind of widespread incompetence, greed, stuupidity, and ignorance makes me sick.
if you can’t blame the accident on someone else, the only other alternative is to say “gee, i’m sorry” and go on doing the same thing until another accident. unfortunately, its a sign of these new times…….
It may be a sign of the times, but that doesn’t make it right. Where are the experienced, competent, trustworthy folks who set safety and compliance standards? We’re regressing in terms of work ethic and common sense.

Oh, I just can’t wait to hear all the comments about how poor BP shouldn’t be held responsible and that any lawsuits are just ridiculous.
