If your car is accelerating out of control, for any reason, why don’t you just turn it off and/or use a combination of downshifting, brakes, and the emergency brake? Why would you try to steer while the car continued to speed up?
Panic would be my guess. I have gotten the pedal of my Toyota stuck because of the floor mat. Luckily I was in the middle of nowhere and it wasn’t that big of a deal. But believe me those options you listed didn’t come to my head – it was get this !#$#$^ mat out of the way.
The floor mat problem sounds a little fishy to me. I had a situation once where my floor mat did slide up and held the accelerator; I fixed it by pulling the mat back with my heel after applying the brake. If that hadn’t worked I would have put the car in neutral.
Some Toyota and Lexus vehicles do not have a “key” and cannot be turned off while the car is being driven. They have a button where the key typically goes and you have to be in park and I believe have the emergency brake pulled to turn the vehicle off.
The floor mats are supposed to be held back by two little pegs which fit in rings just under the driver’s seat. It’s possible for the mat to slip off the pegs. If you have one of the “push to start” cars it is recommended to put it in neutral N gear and apply the breaks.

I have heard this can be a problem.
