Worker Crushed to Death at Ohio Cylinder Plant

A worker has been crushed to death at an Ohio cylinder plant after he got caught between two large rolls of steel.

Worthington Industries in Westerville says 60-year-old Larry Kostelnik died at a hospital after the accident at about 6:30 a.m. Friday.

Deputy Fire Chief Terry Smith tells The Columbus Dispatch that Kostelnik became caught in the equipment and had to be extricated by medics.

The plant has closed for the weekend and will have counselors available on Monday.

The newspaper reports that the company is a global supplier of pressure tanks and cylinders. Kostelnik was a 19-year employee who worked in the receiving department, where materials like steel were delivered for manufacturing.

The company and the Occupational Safety and Health Administration is investigating.
