I wonder how long it takes Minnesota’s OSHA to post reports online. I’d like to see what they found, but if it takes too long to post I’ll forget about it.
Was this a result of misuse of the equipment? Would it have been a preventable accident with proper manufacturers built in safety system? There should be some design liability here. Did the manufacturers printed information warn that this could happen? Did the instructions include the proper and safe use of the activation controls? What company manufactured this product?
