Mich. CIS Summarily Suspends Clawson Dentist’s License on Fraud Charge

The Michigan Department of Consumer & Industry Services (CIS) has summarily suspended the dentistry license of Michael S. Urbanik, D.D.S., due to a felony conviction.

On Oct. 15, 2002, in the 30th Judicial Circuit Court, Ingham County, Michigan, Urbanik was convicted of two felony counts of Medicaid Fraud-False Claim after reportedly admitting to filing Medicaid claims seeking payment for services that he did not provide.

Urbanik was sentenced to 10 days in jail, placed on probation for five years, and ordered to restitution and costs totaling $128,120. Urbanik reportedly failed to notify CIS of his conviction as required by the Public Health Code

On April 3, 2003, the CIS Bureau of Health Services summarily suspended Urbanik’s license pursuant to the Public Health Code, which provides for the mandatory summary suspension of a health professional’s license upon the conviction of a felony.

An administrative hearing will be scheduled to address the status of Urbanik’s suspended license.
