Quite a common incident across the globe, and this causes unnecessary loss of human life / property (especially motor vehicles).

    Queries:

    (i) Why are we not designing a manhole cover which can be removed only by the authorized personnel?

(ii) Can’t iron manhole covers be replaced using a material which has nil salvage / scrap value? Maybe something like ferrocement / porcelain?

What kind of pathetic human garbage would do this? It’s not only the thieves, but those who purchase them. Removing covers puts responsible people in danger. Anyone involved should be shot on sight.
