I wonder what the quality control process is. Since the weapon must be test-fired it needs to be loaded. It would make sense that the procedure is probably something like – load two rounds, fire said rounds, clear the weapon and on to the next. I wonder what additional controls they have put in. It could be another round of inspections (just as error prone since it relies on people). I’m sure there are mechanical devices, x-ray machines and such, that could detect unspent rounds left in the weapon, but the company probably didn’t purchase any equipment. I really want to know more.
And arms dealers should already be clearing any weapon they handle. Never trust that somebody else made the weapon safe; that’s a good way to get shot.
The other thing to consider is that once the weapon is fired, even for testing purposes it should be cleaned. Any gun owner knows that before you start the cleaning process the first thing you want to do is to make sure that there are no shells in the gun. Somehow I suspect that these guns got out without being cleaned.

While this is a huge mistake to ship loaded guns, the number one rule is to treat all guns as loaded.
