N.Y. Contractor Cited in Worker’s Death in Trench Collapse

A contractor has been cited for safety violations following the death of an employee at a Brooklyn site in May.

The U.S. Labor Department’s Occupational Safety and Health Administration says Starpak Contracting faces $25,500 in proposed fines for “willful and serious violations of safety standards” at the site. The worker was killed when the walls of an unprotected 10-foot trench collapsed on him on May 4.

A lawyer for Starpak was not immediately available for comment.

OSHA says the trench lacked any protection against possible cave-ins or a safe way to exit.
